Housing Case Manager

Job Title: Homelessness Prevention and Rapid Re-housing Program (HPRP) Housing Case Manager
Reports To: Program Director, TND-Chattanooga
Mission: Exist to meet the physical, emotional and spiritual needs of women in crisis, equipping them to develop lives of wholeness and hope.
Summary: This new position that is 100% HPRP funded is a full-time position. This person will work with
eligible women exiting jail, prison, drug courts and/or transitional living programs to obtain and sustain appropriate housing. These specialized case managers will be knowledgeable about HPRP, other Recovery Act funds, community-specific resources and develop strong partnerships with local providers to assist with expedited housing. In addition, the HPRP Case Manager, will ensure all participants meet required eligibility determination requirements, take applications for HPRP services and assure that HPRP data collections requirements are satisfied.
Primary Responsibilities and Duties include the following:
- Conduct eligibility screening and complete program intakes of HPRP participants for whom direct services are provided;
- Develop centralized housing plans for each HPRP participant;
- Complete privacy authorizations to share designated HPRP participant information;
- Use adopted standardized forms, transmit completed intake screening, assessment and privacy documents
- Coordinate case-management efforts on behalf of each HPRP participant;
- Maintain a centralized file of all HPRP participant intake, assessment, service delivery notation, case-management, financial assistance notation and privacy records;
- Use the appropriate Homeless Management Information System to enter all HPRP participant intake, assessment, service transaction, financial assistance and case-management data;
- Distribute funds to participants in the form of direct checks to housing providers for permanent housing
- Maintain financial records of HPRP funds received and allocated in support of direct service delivery activities.
- Assure that rent reasonableness test are conducted for each unit
- Work closely with, resource and network with other HPRP providers in the region.
- Additional responsibilities as assigned.
Qualifications:
To be successful in executing the responsibilities and duties assigned in this job, a person must be able to perform each essential function satisfactorily. Education and Experience:
- Bachelor degree preferred or equivalent experience in the local housing industry
- Proven ability to develop solution for women in crisis
- Previous experience in case management with target population preferred
Probation Period Required:
90 days
Other Requirements:
1. Prioritize and manage multiple projects simultaneously.
2. Possess computer skills including e-mail, Word, and Excel.
3. Valid driver’s license.
4. Personal automobile insurance.
